Amends the Pharmacy Practice Act to allow pharmacists to deliver prescription drugs to a patient's agent, including nurses and physician assistants providing hospice or home health services, at the patient's residence, workplace, or medical facility.

Adds definitions for "home health services," "hospice patient," and "hospice services" by cross-referencing the Home Health, Home Services, and Home Nursing Agency Licensing Act and the Hospice Program Licensing Act.

Amends the Illinois Controlled Substances Act to permit possession of controlled substances prescribed for hospice or home health patients by nurses and physician assistants providing such services without requiring separate registration.

Clarifies that nurses (advanced practice, practical, or registered) and physician assistants licensed under their respective practice acts may lawfully possess controlled substances when delivering them as part of hospice or home health care.
