Requires group health insurance policies covering more than 25 employees that provide pregnancy-related benefits to include coverage for diagnosis and treatment of infertility, including in vitro fertilization, embryo transfer, artificial insemination, and other assisted reproductive procedures.

Limits coverage for in vitro fertilization and related procedures to individuals who have failed to achieve or maintain pregnancy through less costly, medically appropriate infertility treatments already covered under the policy.

Caps coverage at 4 completed oocyte (egg) retrievals, with an allowance for 2 additional retrievals if a live birth results from a prior retrieval.

Requires procedures to be performed at medical facilities meeting American College of Obstetric and Gynecology or American Fertility Society standards.

Exempts religious institutions, organizations, and entities sponsored by religious organizations from the coverage requirement if the procedures conflict with their religious and moral teachings.
