Requires circuit court clerks to notify the Department of State Police's FOID department immediately when a person is adjudicated with a mental disability or involuntarily admitted, and to forward a copy of the court order to the Department.

Mandates circuit court clerks to submit semi-annual notifications (July 1 and December 30 each year, beginning July 1, 2016) to the FOID department confirming whether any mental disability adjudications or involuntary admissions occurred in the preceding 6 months.

Requires the Department of Human Services to report information about mental health facility patients to the Department of State Police to determine if individuals are disqualified from receiving or retaining a FOID card or purchasing weapons.

Mandates physicians, clinical psychologists, qualified examiners, law enforcement officials, and school administrators to notify appropriate agencies within 24 hours if they determine a person poses a clear and present danger or has a developmental disability.

Provides immunity from criminal, civil, and professional liability for those making or not making required notifications, except in cases of willful or wanton misconduct.
