Extends the discontinuation date of the Illinois Lottery "Quality of Life" scratch-off game from December 31, 2017 to December 31, 2025.

Net revenue from the Quality of Life game is deposited into the Quality of Life Endowment Fund for appropriation to the Department of Public Health for HIV/AIDS prevention education and grants to public or private entities serving high-risk populations.

Grant funding is allocated by organizational size: 50% to organizations with budgets of $300,000 or less, 25% to organizations with budgets of $300,001-$700,000, and 25% to organizations with budgets exceeding $700,001.

Grants may not be used for institutional, organizational, or community-based overhead costs, indirect costs, or levies, and are intended to augment rather than replace existing State HIV/AIDS funding.

The Department must consult with the Quality of Life Board on game design, promotion, and grant award decisions.
