Amends Section 3-697 of the Illinois Vehicle Code to authorize Chicago Police Memorial Foundation license plates for active/retired law enforcement officers, their family members, surviving family members of deceased officers, and Foundation members or donors

Requires the Secretary of State to create a motorcycle version of the special registration plates in addition to existing passenger vehicle and motor vehicle plates

Charges a $25 fee for original plate issuance ($10 to Chicago Police Memorial Foundation Fund, $15 to Secretary of State Special License Plate Fund) and $25 for each renewal ($23 to Foundation Fund, $2 to administrative fund)

Establishes the Chicago Police Memorial Foundation Fund to support memorial maintenance, annual commemorations, scholarships for children of fallen/catastrophically injured officers, financial assistance to officers and families, and insurance premiums for terminally ill officers
