IL SB2514

Creates the Patient Intake Form Military Veteran Inquiry Act requiring health care providers to include a specific question on patient intake forms asking "Have you or your spouse ever served in the military?"

Applies to all physicians and public or private facilities providing preventive, diagnostic, therapeutic, rehabilitation, mental health, or intellectual disability services on an inpatient or outpatient basis.

Defines "military veteran" to include anyone who served in any component of the U.S. Armed Forces, reservists, National Guard members of any state or U.S. territory.

Establishes the purpose of improving the tailoring of treatment options for military veterans by identifying them during the patient intake process.
