Exempts active, retired, or honorably discharged members of the United States Armed Forces, Illinois National Guard, or Illinois Naval Militia from jury service upon request.

Requires county boards, jury administrators, or jury commissioners to permanently exclude qualifying servicemembers from all current and subsequent jury lists upon request.

Mandates creation and maintenance of a list of persons permanently excluded from jury duty under this provision.

Requires notification to individuals when they are permanently excluded from all current and subsequent jury lists.

Removes reference to "active duty in the Illinois National Guard or Illinois Naval Militia" from existing hardship exemption language in Section 10.2.
