Authorizes a new "continuum of care license" for organizations providing comprehensive services to adults with developmental disabilities, including residential living arrangements, employment opportunities, training programs, and medical facilities.

Requires the Secretary of Human Services to establish licensing standards and rules within 12 months, with facilities providing all required programs and services under a single coordinated license.

Mandates facilities develop a continuum of care plan demonstrating how they will facilitate seamless transitions between service levels, maximize employment opportunities, and provide informed choice to beneficiaries based on individual preferences and needs.

Requires reimbursement to be budget-neutral, with facilities entering into a single provider agreement and receiving no more or less than what comparable facilities received prior to the continuum of care license.

Authorizes the Director of Healthcare and Family Services to request a federal waiver or state plan amendment under Title XIX of the Social Security Act to implement the alternative licensure and reimbursement model for continuum of care facilities.
