Removes the Illinois Optometric Practice Act of 1987 from the January 1, 2017 sunset date and extends the Optometric Practice Act of 1984 sunset to January 1, 2027.

Expands optometrist scope of practice to permit probing and dilation of lacrimal ducts, removal of superficial foreign bodies and eyelashes, removal of superficial lesions and conjunctival cysts, corneal debridement, and corneal shaping with external devices like contact lenses.

Authorizes optometrists meeting educational requirements established by the Department to administer ocular pharmaceutical agents by subcutaneous, subconjunctival, and intramuscular injection, but prohibits retrobulbar, intraocular, and botulinum injections.

Requires applicants and licensees to notify the Department of address changes within 14 days and establishes confidentiality protections for investigative information, with formal complaints remaining public records.

Modifies disciplinary procedures to allow negotiated consent orders at any point in investigations and clarifies notice requirements to use applicants' or licensees' addresses of record.
