Amends the Developmental Disability and Mental Disability Services Act to establish Article 12 concerning community-based services for persons with developmental disabilities.

Allows adults over 18 with developmental disabilities who qualify for Medicaid Waiver services to transition to community-based services or placements if they affirmatively request such services, including those in intermediate care facilities with 9 or more residents or in family homes.

Establishes a transition schedule with 500 persons beginning transitions on June 15 of each year from 2016 through 2020 (2,500 total), and remaining eligible persons transitioning at a reasonable pace thereafter.

Designates the Department of Human Services to oversee the transition process for eligible individuals.

Specifies that participation is voluntary for individuals and providers—no person can be forced to move against their will, and providers cannot be forced to close beds or sign downsizing agreements.
