Creates the Mental Health Income Tax Checkoff Fund as a special fund in the State treasury to be administered by the Department of Public Health.

Authorizes the Department of Public Health to use Fund moneys to make grants to providers of mental health services in Illinois.

Requires the Department to print a checkoff provision on individual income tax forms for taxable years ending on or after December 31, 2017, allowing taxpayers to contribute a minimum of $1 to the Mental Health Income Tax Checkoff Fund.

Specifies that taxpayer contributions will reduce their refund or increase their payment owed, and that failure to remit increased payment will reduce the contribution accordingly.

Takes effect immediately upon becoming law.
