Secretary of State shall issue a Voter Identification Card to registered voters who lack acceptable photo identification, containing the voter's name, signature, photograph, state seal, and current residence address.

Acceptable forms of photo identification for voting include Illinois Driver's License, State Identification Card, FOID Card, U.S. Passport, and any government-issued identification with current photo and address.

Voters applying for a Voter Identification Card must provide photo or non-photo identity documents, proof of voter registration, documentation of name and address, and other specified documents.

Indigent voters (at or below 125% of federal poverty guidelines) and those with religious objections to being photographed may vote a provisional ballot and sign an affidavit claiming an exemption instead of presenting photo identification.

Requires photo identification verification for voting in person on election day, during early voting, and when voting by mail if registered by mail, with exceptions for eligible exemptions.
