Creates the Prostate Cancer Awareness Fund as a special fund in the State treasury to receive fees from a new special license plate.

Authorizes the Illinois Department of Public Health to issue decals for Universal special license plates dedicated to prostate cancer awareness.

Sets special license plate fees at $25 for original issuance ($10 to Prostate Cancer Awareness Fund, $15 to Secretary of State Special License Plate Fund) and $25 for renewal ($23 to Prostate Cancer Awareness Fund, $2 to Secretary of State Special License Plate Fund).

Requires all moneys collected in the Prostate Cancer Awareness Fund to be paid as grants to the Prostate Cancer Foundation of Chicago.

Allows the special plates to be issued on passenger vehicles and motorcycles through the multi-year registration procedure established in existing vehicle code law.
