Amends Section 512 of the Illinois Income Tax Act to require the Department of Revenue to print a county designation on individual income tax return forms for tax years beginning after December 31, 2016.

Maintains existing provisions requiring taxpayers for tax years 1986-1995 to indicate their high school or unit school district on tax forms, with failure to do so not invalidating the return.

Continues the requirement that the Department provide the State Board of Education with individual income tax receipt information by school district from the Geographic Information System for tax years ending December 31, 1995 and thereafter.

Takes effect immediately upon becoming law.
