Amends Section 19-3 of the Illinois Election Code to establish requirements for vote by mail ballot applications.

Allows applicants to submit applications electronically without requiring a signature, only requiring them to mark a certification box stating the information is true and correct.

Requires that vote by mail ballot applications sent to post office boxes controlled by non-election authorities include a valid phone number and be turned over to the appropriate election authority within 7 days, or within 2 days if received within 2 weeks of the election.

Establishes a $100 per application fine for failure to timely transfer applications to election authorities, with removal or tampering with postmarks creating a rebuttable presumption of violation.

Permits mail-in ballots postmarked by election day to be counted during the provisional ballot counting period, which extends through the 14th day following election day.
