Creates the Thriving Youth Income Tax Checkoff Fund as a special fund in the State treasury to support community-based youth programs.

Allows Illinois taxpayers to voluntarily contribute to the Fund on their individual income tax returns by designating a contribution amount of at least $1.

Directs the Department of Human Services to use Fund moneys to make grants to providers delivering non-Medicaid services for community-based youth programs in Illinois.

Requires the Illinois Department of Revenue to print the checkoff provision on standard individual income tax forms for taxable years ending on or after December 31, 2017.

Takes effect immediately upon becoming law.
