Changes Illinois organ donor registry from opt-in to opt-out system, requiring applicants for driver's licenses or identification cards (age 18+) to affirmatively exclude themselves rather than opt in to become donors.

Secretary of State must offer applicants the opportunity to have their name excluded from the First Person Consent organ and tissue donor registry and must provide a brochure explaining the registry and donation process.

Individuals who do not exclude themselves are deemed to have given full legal consent to organ and tissue donation upon death, with consent effective without witness signatures.

Allows organ procurement agencies, tissue banks, and county coroners/medical examiners to inquire whether a potential donor is listed in the registry.

Takes effect immediately upon becoming law.
