• Disclosure must include: a statement indicating the entity is a service provider or contractor, the contractor's name, the contractor's state or federal tax designation, and the contractor's principal place of business.

• Violations constitute a business offense with a $1,000 fine per occurrence, enforceable by circuit court in the county where the violation occurs.

• Limits home rule powers by preventing home rule units from regulating in a manner inconsistent with these requirements.

• Effective immediately upon becoming law.
