Amends the Illinois Public Aid Code to require applicants and recipients of SNAP or state medical assistance benefits to prepare and submit a personal employment and self-sufficiency plan at application or eligibility redetermination.

Imposes work requirements for single parents heading an assistance unit who are able to work: minimum 30 hours per week to receive SNAP or medical assistance benefits.

Imposes work requirements for two parents heading an assistance unit: combined minimum 35 hours per week to receive SNAP or medical assistance benefits.

Requires the Department of Human Services and Department of Healthcare and Family Services to seek any necessary federal waivers to implement these work requirement provisions.

Implementation is subject to federal approval.
