Requires SNAP and state medical assistance applicants and recipients to prepare and submit a personal employment and self-sufficiency plan at application or eligibility redetermination, subject to federal approval.

Mandates single parents heading an assistance unit who are able to work must work or participate in work activities for at least 30 hours per week to receive SNAP or medical assistance benefits.

Requires two parents heading an assistance unit to work a combined 35 hours per week to receive SNAP or medical assistance benefits.

Directs the Department of Human Services and Department of Healthcare and Family Services to seek any necessary federal waivers to implement these work requirement provisions.
