Requires Secretary of State to issue Voter Identification Cards to registered voters without acceptable government-issued photo identification, including voter's name, signature, photograph, state seal, and current residence address.

Defines acceptable forms of photo identification for voting as Illinois Driver's License, State ID Card, Disabled Person ID, Senior Citizen ID, FOID Card, U.S. Passport with current address, or other government-issued ID with name, photo, and current address.

Mandates any person desiring to vote present either a government-issued photo identification card or a Voter Identification Card to election judges for identity verification before receiving a ballot.

Provides exemptions for indigent voters (income at or below 125% of federal poverty guidelines) and those with religious objections to being photographed, allowing them to vote provisional ballots by signing an affidavit.

Establishes procedures for provisional ballot voting with required documentation including photo identity or non-photo documents (birth certificate, voter registration card, naturalization document, tax returns, Social Security statements, etc.) and proof of address (utility bills, bank statements, rental agreements, property documents, W-2 forms).
