Creates the Alzheimer's Research Tax Checkoff Fund as a special fund in the State treasury to be administered by the Department on Aging.

Allows Illinois taxpayers to contribute to the Fund in voluntary amounts of $1 or more on their state income tax returns for taxable years ending on or after December 31, 2017.

Directs Fund moneys to be used for grants to associations and entities that conduct Alzheimer's disease research, provide services to individuals and families affected by Alzheimer's disease, or both.

Requires the Department to print a provision on standard individual income tax forms indicating taxpayers can contribute and that contributions will reduce their refund or increase their payment amount.

Takes effect immediately upon becoming law.
