Candidates who changed their name within 3 years before filing their nomination petition must list all prior names and dates of change on the ballot or petition, with failure to do so grounds for ballot removal or denial of certification.

Exceptions to the name change disclosure requirement apply to name changes from adoption, marriage, or dissolution of marriage.

Candidates are not required to use a spouse's surname after marriage or return to a previous surname following divorce or marriage dissolution.

Candidates may not use political slogans, titles, degrees, or nicknames implying professional status as part of their ballot name; political slogan is defined as any words conveying meaning other than the candidate's personal identity.

Election authorities and the State Board of Elections must remove or refuse to certify any candidate name designations that violate these requirements, with candidates able to seek relief in circuit court if their name is removed.
