Department of Children and Family Services must assist youth in care in obtaining documents necessary for independent adult living before case closure and wardship termination.

Required documents include state identification, Social Security card, medical records, birth certificate, educational records, voter registration, immigration documents if applicable, and a life book or personal history compilation.

Department must provide youth with education on executing a healthcare power of attorney and supply a list of community resources including the Midwest Adoption Center and Illinois Chapter of Foster Care Alumni of America.

Court must find it is in a minor's best interest to continue wardship if the Department has not made reasonable efforts to ensure the minor has necessary documents for adult living as defined in the bill.

When terminating wardship for minors over 18, court must ensure the Department has provided documents necessary for adult living, and the youth must receive a copy of the court's termination order.
