Department of Corrections must collect and report violence data per 100 committed persons, including assaults, weapon use, sexual assault, suicide attempts, and segregation placements, as defined under 20 Ill. Adm. Code 504.

Department must report public safety data including persons released directly from restrictive housing to community, housing types upon release, completion of evidence-based programs (educational, vocational, chemical dependency, sex offender treatment, cognitive behavioral), and parole revocation rates by county.

Director of Corrections and facility executive teams must review violence data quarterly, identify trends, develop action items to mitigate violence, and establish committees at each facility to develop action plans with strategies to incentivize good conduct.

All collected data must be included in Department's quarterly reports to the General Assembly with aggregate agency-level charts and individual facility reports, filed electronically with the House Clerk and Senate Secretary.

Department must post the quarterly report to its website at the time of submission to the General Assembly.
