Establishes a Community Care Program Medicaid Initiative requiring the Department on Aging to provide targeted funding to care coordination units to help seniors complete medical assistance applications and make improvements to the application process.

Creates the Community Care Program Medicaid Enrollment Oversight Task Force with 23 members including state agency representatives, care coordination units, labor unions, seniors advocates, and legislative appointees to oversee the Initiative and meet quarterly.

Requires the Department on Aging to collect and publish data on Community Care Program participants enrolled in Medicaid, those not enrolled but eligible, and those receiving services without any Medicaid coverage, categorized by Planning and Service Area.

Establishes payment incentives of at least $300 per completed medical assistance application for care coordination units that meet or exceed their 2017 baseline monthly application completion rates.

Directs the Department of Human Services to adopt a uniform application submission process within 60 days and coordinates three state departments to create a streamlined enrollment process with consolidated forms accepted across all agencies.

Terminates the Initiative 5 years after the effective date, at which point the Task Force shall dissolve.
