Amends the Mental Health and Developmental Disabilities Administrative Act to require annual health risk screening assessments for individuals residing in community integrated living arrangements.

Mandates transition to a web-based, electronic screening tool for annual client assessments beginning July 1, 2019, replacing paper-based assessments.

Allows providers to request exceptions to use paper-based assessments with written justification submitted to the Department.

Authorizes the Department to contract with third-party entities to create and implement the electronic screening tool, subject to appropriations.

Requires the Department to update Illinois Administrative Code rules to incorporate the web-based, electronic assessment tool.
