Amends Section 2-200 of the Mental Health and Developmental Disabilities Code to require facilities to post contact information for the Guardianship and Advocacy Commission and the Protection and Advocacy for Persons with Developmental Disabilities agency in public areas.

Requires facilities to inform recipients age 12 and older and parents or guardians of their right to designate a person or agency to receive notices about the recipient or to prohibit disclosure of information.

Mandates that facilities contact at least two people designated by the recipient (such as family, attorney, or advocate) to inform them of the recipient's location at the facility.

Requires facilities to inform recipients of circumstances permitting emergency forced medication, electroconvulsive therapy, restraint, or seclusion, and to document the recipient's preference regarding which intervention they would prefer.

Specifies that facilities must give due consideration to the recipient's stated preferences regarding emergency interventions when such circumstances arise.
