Department of Corrections shall provide persons upon release (parole, mandatory release, final discharge, or pardon) with a voting rights information form created by the State Board of Elections.

State Board of Elections must create and post on its website a voting rights information form tailored to formerly incarcerated persons containing information on eligibility to vote, voting locations, voter registration, and automatic voter registration.

Form distribution applies to all persons released from Department of Corrections facilities regardless of release type.

State Board of Elections responsible for form design and website posting; Department of Corrections responsible for distributing forms to released individuals.
