Amends the Illinois Income Tax Act to create an income tax checkoff allowing taxpayers to voluntarily contribute to qualified veterans home funds starting with taxable years ending on or after December 31, 2018.

Requires the Department to print a provision on standard individual income tax forms indicating taxpayers may contribute to a qualified veterans home fund with a minimum contribution of $1.

Allows taxpayers to designate contributions to one or more of four qualified veterans home funds: LaSalle Veterans Home Fund, Anna Veterans Home Fund, Manteno Veterans Home Fund, or Quincy Veterans Home Fund.

Contributions reduce the taxpayer's refund or increase the amount of payment owed with the return; failure to remit any increased payment reduces the contribution accordingly.

Takes effect immediately upon becoming law.
