Allows group and individual health insurance policies and managed care plans to provide coverage for recovery housing services for persons with substance use disorders who are at risk of relapse after discharge from treatment facilities.

Requires an individualized assessment using American Society of Addiction Medicine criteria to determine if a person is at risk of relapse and needs supportive services, considering history of poverty, job insecurity, and lack of safe housing.

Recovery housing must be administered by a community-based agency licensed by or under contract with the Illinois Department of Human Services, Division of Substance Use Prevention and Recovery.

Coverage may include substance use disorder treatment services, transitional housing with meal plans, case management and referral services, and job training or placement services.

Allows insurers to rate community-based agencies based on their ability to reduce health care costs, lower recidivism rates, improve outcomes, track persons with substance use disorders, and improve quality of life through recovery, education, employment, and housing services.
