Amends Illinois Code of Civil Procedure Section 21-103 to allow courts to waive publication requirements for name changes when a person files a written declaration that publishing the notice would put them at risk of physical harm or discrimination.

Requires the person seeking a waiver to provide evidence supporting their claim that publishing notice of the name change would create risk of physical harm or discrimination.

Maintains existing exemption allowing waiver of publication requirements in minor name changes when reasonable notice and opportunity to be heard is provided to parents and custodians.

Preserves existing authority for the Director of State Police to request waiver of publication requirements to protect witnesses in criminal investigations or proceedings.

Caps publication fees at the lowest classified rate charged to commercial users, including all available cash discounts and multiple insertion discounts.
