Amends the Developmental Disability and Mental Disability Services Act to create Article 12 establishing a community-based services transition program for adults with developmental disabilities.

Applies to adults over 18 with developmental disabilities who qualify for Medicaid Waiver services, reside in intermediate care facilities with 9+ residents or family homes, and affirmatively request community-based services or placement.

Department of Human Services oversees transitions according to schedule: 500 persons begin transitioning by June 15 each year from 2017 through 2021, with remaining persons transitioning at reasonable pace thereafter.

Participation is voluntary; the law does not force individuals who decline community-based services to move, nor require providers to close beds or enter downsizing agreements against their will.

Takes effect immediately upon becoming law.
