Prohibits state employee health benefit programs from charging higher copayments, coinsurance, or office visit deductibles for physical therapist services than for primary care physician or osteopath office visits.

Requires insurers to clearly disclose physical therapy coverage availability and all related limitations, conditions, and exclusions in policy documents.

Extends the same physical therapist cost-sharing restrictions to all health insurance plans under the Illinois Insurance Code.

Mandates the Commission on Government Forecasting and Accountability to conduct an actuarial analysis of the cost impact on health carriers, insureds, and other public and private payers, with findings due by December 31, 2019.
