Extends the Physician Assistant Practice Act from January 1, 2018 repeal date to January 1, 2028 by transferring it from the regulatory sunset provision to a new extended sunset date.

Changes terminology throughout the Physician Assistant Practice Act from "supervising physician" to "collaborating physician" to reflect a more collegial relationship model.

Authorizes physician assistants in hospitals, hospital affiliates, and ambulatory surgical treatment centers to prescribe Schedule II controlled substances (in addition to III-V) under specified conditions including maximum 30-day supplies and monthly physician consultation.

Requires physician assistants to verbally identify themselves to each patient and prohibits use of "doctor" or "physician" titles; establishes 50 hours of continuing education per 2-year renewal cycle.

Adds physician assistants as eligible providers under Illinois Medicaid (Public Aid Code) and updates controlled substances act definitions to recognize physician assistants and "mid-level practitioners" for prescriptive authority purposes.
