Adds "sexual orientation" definition to the Gubernatorial Boards and Commissions Act, with the same meaning as in the Illinois Human Rights Act.

Requires the Governor's Office of Boards and Commissions to include an optional sexual orientation disclosure field in uniform board and commission applications for reporting purposes.

Mandates the Governor's annual demographic reports to the General Assembly include voluntarily disclosed sexual orientation information alongside ethnicity, gender, and disability status for appointees and applicants.

Applies sexual orientation reporting requirements to all three categories of demographic information: individual appointments made, aggregate data on applicants not appointed, and overall demographic composition of gubernatorial appointees.
