Adds licensed optometrists to the list of healthcare providers whose medical procedures minors can consent to without parental permission, including married minors, parents who are minors, pregnant minors, and anyone 18 years or older.

Extends consent authority for primary care services (screening, counseling, immunizations, medication, and treatment) to minors aged 14-17 living separately from parents/guardians when identified in writing by an adult relative, homeless service agency, attorney, school liaison, social service agency, or religious organization.

Requires healthcare professionals to reasonably believe the minor understands the benefits and risks of proposed care, and provides liability protection if they rely in good faith on the minor's representations or written identification.

Maintains confidentiality of minor-provider communications and protects the minor's existing legal rights under federal, state, and local law, including the Parental Notice of Abortion Act of 1995.

Allows parents who are minors to consent to medical and dental procedures for their children with the same legal capacity as adults.
