Creates the Thriving Youth Income Tax Checkoff Fund as a special fund in the State treasury for the Department of Human Services to make grants to providers delivering non-Medicaid community-based youth programs.

Requires the Department to print a checkoff provision on individual income tax forms for taxable years ending on or after December 31, 2017, allowing taxpayers to contribute a minimum of $1 to the Thriving Youth Income Tax Checkoff Fund, with contributions reducing refunds or increasing payment amounts.

Creates an Illinois Police Memorial checkoff on income tax forms allowing taxpayers to contribute a minimum of $1 to the Criminal Justice Information Projects Fund for taxable years ending on or after December 31, 2017.

Directs contributions to the Criminal Justice Information Projects Fund to be distributed equally among the Chicago Police Memorial Foundation Fund, the Police Memorial Committee Fund, and the Illinois State Police Memorial Park Fund for memorials, scholarships, and financial assistance to police officers and their families.
