IL SB2296

Establishes the Advance Directives Registry under the Department of Public Health to allow Illinois residents to submit advance directive documents including Living Will Declarations, Powers of Attorney for Health Care, Mental Health Treatment Forms, and POLST forms.

Makes registry information available to hospitals licensed under the Hospital Licensing Act or organized under the University of Illinois Hospital Act, which may rely on registry documents as accurate copies.

Requires the Department of Public Health to adopt necessary rules, publish Spanish-language versions of advance directive forms, and provide website information about registry use.

Provides immunity from civil and criminal liability for the Department and its employees in connection with creating and maintaining the registry, except in cases of gross negligence or willful misconduct.

Establishes unauthorized submission of documents to the registry as a Class A misdemeanor.
