IL SB2424

Establishes a Diversion from Facility-based Care Pilot Program consisting of at least 6 homes in various locations to serve individuals with intellectual or developmental disabilities currently receiving Home and Community Based Services (HCBS) who are at risk of facility-based care due to significant behavioral challenges.

Limits each home to no more than 4 residents with individual bedrooms, regulated according to community-integrated living arrangement guidelines, and serves individuals for a period ranging from 1 to 2 years or longer if appropriate.

Requires providers to have adequate resources, experience, and qualifications, and mandates that opportunities for participation in HCBS services be offered to residents as they become available.

Requires the Department to develop data collection and reporting requirements in collaboration with providers and submit annual reports to the General Assembly and Governor beginning December 31, 2020 outlining the program's progress and effectiveness.

Sunsets the program on January 1, 2023, with an effective date of July 1, 2019.
