Removes the June 30, 2019 repeal date for the Healthy Local Food Incentives Program, making the program permanent.

Requires the Department of Human Services to establish a program that doubles the purchasing power of SNAP (food stamp) benefits at participating Illinois farmers markets for eligible fruits and vegetables.

Authorizes an annual grant of $500,000 to a qualified nonprofit organization to distribute matching dollar incentives to participating farmers markets based on SNAP spending.

Requires the nonprofit administrator to submit progress reports within 90 days after each grant cycle, including information on participating markets, funds distributed, transaction volume, and program impact on fresh produce consumption.

Mandates that 100% of deposited funds be distributed to participating Illinois farmers markets for healthy food incentives, with the Department of Human Services required to adopt implementation rules by December 31, 2017.
