Amends the Uniform Health Care Service Benefits Information Card Act to include dental plans in the requirement to issue uniform health care benefit information cards.

Adds "dental plan" definition to mean any entity providing coverage for dental care services, including entities subject to the Dental Service Plan Act.

Requires health benefit plans offering dental coverage and dental plans to include a statement on their information cards indicating whether they are subject to regulation by the Department of Insurance.

Applies uniform card requirements to dental plans, including mandatory data elements on front (processor control number, group number, card issuer identifier, cardholder ID number, cardholder name) and back (claims submission information and help desk telephone numbers).

Requires new cards to be issued upon enrollment and reissued whenever coverage changes affect the mandatory data elements displayed on the card.
