Amends Illinois Public Aid Code Section 5-5.4h to change reimbursement terminology from "long-term care facilities for persons under 22 years of age" to "medically complex for the developmentally disabled facilities licensed under the MC/DD Act."

Establishes tiered per diem reimbursement rates for exceptional care starting April 1, 2019: Tier 1 at $326 for residents receiving 51% or more caloric intake via feeding tube, Tier 2 at $546 for tracheostomy care without ventilator, and Tier 3 at $735 for tracheostomy care with ventilator.

Maintains expedited 30-day Medicaid reimbursement schedule for facilities meeting criteria of serving exceptional care patients with 30% or more receiving ventilator care through March 31, 2019.

Transfers reimbursement calculations and direct payment responsibility from Department of Human Services to Department of Healthcare and Family Services effective April 1, 2019, including shift of appropriations between departments.

Requires facilities to document complex medical conditions in resident medical records using Minimum Data Set standards instead of submitting MDS assessments to the Department, and establishes that specified reimbursement rates serve as minimum amounts.
