Creates an Office of Inspector General within the Department of Human Services to investigate allegations of mental abuse, physical abuse, sexual abuse, neglect, and financial exploitation at mental health and developmental disabilities facilities and community agencies.

Inspector General is appointed by the Governor with Senate confirmation for 4-year terms and operates independently from the Department with separate appropriations.

Establishes mandatory reporting requirements: employees must report allegations within 4 hours and deaths within 24 hours to the Inspector General hotline; failure to report is a Class A misdemeanor.

Requires facilities and agencies to submit written responses within 30 days of substantiated findings or recommendations, with implementation reports due every 30-60 days until completed; allows requests for clarification and reconsideration of findings through multi-layer review.

Creates a 7-member Quality Care Board to oversee the Inspector General's operations and establishes reporting to the Health Care Worker Registry for substantiated findings of physical abuse, sexual abuse, financial exploitation, or egregious neglect.
