Urges the Department of Children and Family Services (DCFS) to develop a plan in collaboration with private agencies to create a review system for any child entering their third placement in substitute care.

Requires DCFS to submit a report no later than December 31, 2017 with recommendations to the General Assembly on how the review system should be developed and implemented.

Cites that in 2014, 20.7% of children entering substitute care had at least three different placements in their first year.

Notes that children experiencing multiple placements have higher rates of behavioral and mental health problems, educational difficulties, and juvenile delinquency.

States that youth with minimized placement changes experience fewer school changes, less trauma, improved mental health outcomes, and greater likelihood of academic achievement.
