Creates the Mental Health Income Tax Checkoff Fund as a special fund in the State treasury to be administered by the Department of Public Health.

Authorizes the Department of Public Health to use Fund moneys to make grants to mental health service providers in Illinois.

Requires the Department to print a checkoff provision on standard individual income tax forms for taxable years ending on or after December 31, 2019, allowing taxpayers to contribute a minimum of $1 to the Mental Health Income Tax Checkoff Fund.

Specifies that taxpayer contributions will reduce their refund or increase the amount of payment owed, and that failure to remit increased payment reduces the contribution accordingly.

Takes effect immediately upon becoming law.
