Requires the Department of Public Health to create and maintain a list of immunizing agents derived from aborted fetal tissue and identify alternative immunizing agents when available.

Updates the list annually and makes it publicly available on the Department's website.

Requires health care providers to notify individuals (or parents/guardians for children) before administering vaccines derived from aborted fetal tissue, effective January 1, 2020.

Mandates health care providers offer alternative immunizing agents when available and inform individuals of their option to decline the vaccine.

Defines "health care provider" as physicians, advanced practice registered nurses, physician assistants, hospital facilities, and other licensed health care service providers.
