Extends the Lake County Children's Advocacy Center Pilot Program through December 31, 2021, allowing jurors in Lake County to voluntarily donate their juror fees to the center.

Requires jurors electing to donate fees to complete a form with a statement in at least 14-point bold type indicating that donation is optional.

Mandates that all donated fees be transferred by the county to the Lake County Children's Advocacy Center at the same time non-donating jurors receive payment, and that funds be used exclusively for center operations.

Requires the Lake County board to report annually to the General Assembly and Governor on juror participation rates, total donations collected, fund usage, and program implementation costs through July 1, 2021.

Repeals this section on January 1, 2022, ending the pilot program.
