Department of State Police must revoke a Firearm Owner's Identification (FOID) Card for one year if the card holder reports firearms lost or stolen in 3 separate incidents within a 2-year period

Law enforcement agencies must forward information to the Department of State Police when they learn a card holder has reported 3 separate lost/stolen firearm incidents within 2 years

An "incident" counts as one occasion of loss or theft regardless of how many firearms were involved; if police recover an unreported lost/stolen firearm, that recovery also counts as an incident

Attempting to acquire a firearm using a revoked FOID Card is classified as a Class 4 felony

Amends both the Firearm Owners Identification Card Act and the Criminal Code of 2012
