Secretary of State may issue special Prostate Cancer license plates to eligible applicants in the form prescribed by the Secretary.

Plates may only be affixed to passenger vehicles of the first division and motor vehicles of the second division weighing no more than 8,000 pounds.

Original issuance fee is $25, with $10 deposited to the Prostate Cancer Awareness Fund and $15 to the Secretary of State Special License Plate Fund for administrative costs.

Renewal fee is $25 per registration period, with $23 deposited to the Prostate Cancer Awareness Fund and $2 to the Secretary of State Special License Plate Fund.

Plates may be issued as vanity or personalized plates at the Secretary's discretion and expire according to the multi-year procedure established in Section 3-414.1 of the Illinois Vehicle Code.
